"Inzwischen bin ich überzeugter 'Psychosomat'" - Kasuistik einer jungen Erwachsenen mit Diabetes mellitus Typ 1 und ausgeprägter hyporektischer Ess-Störung

Abstract

Wir berichten über eine 19-jährige Patientin mit Diabetes mellitus Typ 1 und einer ausgeprägten hyporektischen Ess-Störung, die wiederholt unter lebensbedrohlichen Blutzuckerentgleisungen litt. Anhand der Kasuistik soll exemplarisch gezeigt werden, inwieweit beide Krankheitsbilder ineinander greifen, sich gegenseitig beeinflussen und wie zwei in der operationalisierten psychodynamischen Diagnostik (OPD) beschriebene zentrale Konflikte in dieser Doppelerkrankung Ausdruck finden bzw. von den PatientInnen zur Konfliktbearbeitung funktionalisiert werden. Bei der psychotherapeutischen Behandlung dieser in Kombination auftretenden Erkrankungen ist zu beachten, dass die TherapeutInnen die widersprüchlichen und ambivalenten Bedürfnisse der PatientInnen aushalten müssen und gleichzeitig nicht zu deren Spielball werden dürfen. Ein klares, abgegrenztes und mit den PatientInnen immer wieder zu besprechendes, aber nicht zu diskutierendes Behandlungsregime kombiniert mit einer auf Dialog, Fürsorge und Verständnis basierenden Psychotherapie ist notwendig, um mit diesen PatientInnen konstruktiv arbeiten zu können.

We report about a 19-year-old female patient with diabetes mellitus type 1 and severe anorexia, who presented with life-threatening unstable metabolic imbalance. It is illustrated how these two illnesses interfere and influence each other and how they may be used to express two central conflicts, which are described in the Operationalizing Psychodynamic Diagnoses (OPD). This paper describes in particular the significant role of the psychotherapist who has to manage the conflicting emotions of the patient. Furthermore, the dilemma in the treatment is also discussed. On the one hand there is the attitude that treatment should be based on dialogue, care and understanding, yet on the other hand the treatment options should not be discussed with the patients.

El estudio se centra en el caso de una joven diabética de 19 años con diabetes mellitus tipo 1 que sufre de marcados trastornos alimenticios hiporécticos, lo que conlleva peligrosos deslices en su nivel de azúcar en la sangre. Gracias a la casuística se muestra ejemplarmente, cómo los dos cuadros clínicos están interrelacionados y cómo dos conflictos centrales, descritos en la diagnóstica psicodinámica operacionalizada, se plasman en esta doble enfermedad, o mejor dicho, cómo estos son usados por los pacientes para asimilar el conflicto. Durante el tratamiento psicoterapéutico de esta enfermedad combinada debe considerarse, que a al terapeuta se le presenta la difícil tarea de, por un lado, soportar las necesidades contradictorias y ambivalentes de las o los pacientes, mas sin convertirse en un títere en las manos de éstos. Así para poder trabajar de manera constructiva con los pacientes, se hace indispensable, combinar un régimen de tratamiento claro y nítido, siempre renegociado pero no discutido con el paciente, y una terapia fundamentada en el diálogo, la caridad y la comprensión.
