%X Objective: To analyze the results of planning postoperative hospital discharge in the elderly. Method: This
was a multiple cases study with a quantitative approach using the following research techniques: semistructured
interviews, observation, documental survey and follow-up by telephone in a sample of 12 elderlies.
Results: Reports of significant improvement in infection control were observed after hospital discharge,
100% of the sample demonstrated the ability to identify at least two risk factors for infection. There was an
increased adherence to the therapeutic regimen with a significant increase of 83.4% of outpatient medical
care and maintenance of the use of prescribed medications. Conclusion: It is considered that the execution of
