Objective: To describe the effect of home care nursing in knowledge, hospitalization and blood pressure levels in patients with hypertension treated impaired. Method: quantitative research with a retrospective design, made from the database extension project developed in a basic health unit of a municipality in the metropolitan region of Curitiba-PR. The activities included in the project were educational group and follow-up at home for 12 months. The sample consisted of 16 users with hypertension. Results: There was a higher frequency of women with a mean age of 63 year old. There was improvement in knowledge about hypertension, reduction in the rate of hospital admission due to hypertensive crisis and increase in the proportion of individuals with controlled blood pressure.
