Die hier vorgestellte Studie über die Hauspflege von Patienten und Patientinnen mit fortgeschrittener Demenz verdeutlicht, auf welche Weise implizite Kontextfaktoren zu einem Bias beim Verstehen der Hauspflege führen können: Die meisten vorliegenden Untersuchungen wurden in "1.-Welt-Ländern" durchgeführt, die sich auszeichnen durch einen hohen Anteil älterer Populationen, einen hohen Arbeitslosenanteil, durch Kleinfamilien und durch Sozial- und Gesundheitssysteme, die Hauspflege unterstützen. Insoweit resultiert der Kernbestand des wissenschaftlichen Wissens über Hauspflege aus den individuellen Erfahrungen der Hauptpflegepersonen, die für die Pflege ihrer Familienangehörigen institutionalisierte Hilfesysteme nutzen können; letztere existieren jedoch kaum in Entwicklungsländern. In diesem Artikel wird deshalb das Konzept der "Pflege in Familien" vorgestellt, das auf die kollektive Dimension der Hauspflege und der hieraus erwachsenden Ressourcen zielt. Das Konzept der "Pflege in Familien" soll zugleich Interventionen aufzeigen, die mit der Wirklichkeit der Pflegenden in Entwicklungsländern und mit deren Alltag besser abgestimmt sind.... view less

The study of home care of patients with advanced dementia illustrates how implicit contextual issues in the literature bias understanding about family care. Many of the studies are conducted in developed countries with an older population, high unemployment, nuclear family structure and with social and health care systems that support caregiving at home. The body of knowledge concerning family care is shaped by individual experiences of primary caregivers that have systems of formal support to care for their relatives, these systems are practically nonexistent in developing countries. This paper presents the concept of "caregiving within the family" that sheds light on the collective dimension of family care and caregivers' resourcefulness. The concept of family care is thus developed enabling interventions that fit with caregivers' realities and everyday life.... view less

La realización de un estudio sobre cuidado en la casa de pacientes con demencia avanzada permite ilustrar como asuntos de contexto implícitos en la literatura, sesgan la comprensión que se tiene sobre el cuidado familiar. La mayoría de los estudios realizados en este campo provienen de países desarrollados con población envejecida, altas tasas de empleo, estructuras familiares nucleares y sistemas médico-sociales de apoyo para el cuidado en la casa. El cuerpo de conocimientos sobre el cuidado familiar esta sustentado en las experiencias individuales de los cuidadores principales que cuentan con sistemas de apoyo. Y esos sistemas son prácticamente inexistentes en países en vías de desarrollo. En este artículo presento el concepto de "Cuidar en familia" que saca a la luz una dimensión colectiva del cuidado familiar y la recursividad de los cuidadores. Al ampliarse el concepto de cuidado familiar se posibilitan intervenciones ajustadas a la realidad y cotidianidad de los cuidadores.... view less
