%X "Demografische und epidemiologische Entwicklungen lassen einen erheblich erhöhten Druck auf die medizinische und pflegerische Versorgung erwarten. Daraus ergibt sich die Dringlichkeit zweier gesellschaftspolitischer Ziele: die Gesundheit der Älteren zu erhalten sowie ihre Potentiale zu nutzen. Da sich gesundheitliche Ungleichheit bis ins hohe Alter fortsetzt, sind sozioökonomische und gesundheitliche Ungleichheit und die Zusammenhänge zwischen ihnen sind wichtige Themen im Zusammenhang mit Prävention, auch im Alter. Im vorliegenden Paper werden daher Eckpunkte für primärpräventive Strategien mit benachteiligten Älteren entwickelt. Der erste Teil des vorliegenden Discussion Papers fasst den derzeitigen Stand zu Gesundheits- und Krankheitsmustern sowie Befunde zur sozial bedingten Ungleichheit der Gesundheitschancen bei älteren Menschen zusammen. Im zweiten Teil werden theoretische und empirische Ansatzpunkte zum Verständnis des Zusammenhangs zwischen sozioökonomischer und gesundheitlicher Ungleichheit aufgezeigt: Bourdieus Konzepte des ökonomischen, kulturellen und sozialen Kapitals sowie Befunde zum Verhältnis von Bildung und gesellschaftlicher Partizipation im Alter. Auf diesem Hintergrund wird ein soziallagensensibles Modell der Kontext- und Prozessfaktoren für aktives und gesundes Altern vorgestellt. Im dritten Teil wird eine Standortbestimmung für geeignete primärpräventive Interventionsstrategien für benachteiligte Ältere unternommen. Der Stadtteil (die Kleinstadt, das Dorf) wird als relevantes Setting für präventive Strategien eingeführt und begründet. Im Anschluss werden Ergebnisse einer im Jahr 2005 durchgeführten Pilotrecherche zum Thema 'stadtteilbasierter Gesundheitsförderung für benachteiligte ältere Menschen' dargestellt: Erfolgreiche Projekte arbeiten zielgruppenspezifisch, aktivierend und partizipativ. Bedürfnisse der Zielgruppen betreffen breite Themenspektren. Die Unterstützung sozialer Vernetzung älterer Menschen gilt als zentral. Erfolgreiche Projekte bauen auch auf lokale Akteursnetzwerke, um Ressourcen im Stadtteil zu erschließen. Evaluation und Qualitätsmanagement befinden sich noch im Anfangsstadium. Abschließend werden Schlussfolgerungen für den bestehenden Forschungsbedarf gezogen." (Autorenreferat)
%X "Health inequalities in old age: Starting points for community-based prevention Maintaining the health of older people and making use of their potentials are urgent social policy priorities in view of demographic trends that can be expected to considerably increase the pressure on medical and nursing services. Since health inequalities persist well into the later years, socio-economic and health inequalities and the interactions between them are important issues regarding prevention, also in old age. This paper explores salient aspects of primary prevention strategies for disadvantaged older people. The first part summarises the current findings on patterns of sickness and health and on social inequality of health opportunities among older people. The second part identifies theoretical and empirical starting points for understanding the connections between socioeconomic and health inequalities: Bourdieu's concepts of economic, cultural and social capital; findings on the relationship between education and social participation in old age; and concepts for context and process factors for active, healthy ageing. On that basis a model of the influencing and process factors of active and healthy ageing is developed that also reflects the socio-economic and cultural conditions. The third part examines the starting points for appropriate primary prevention intervention strategies for disadvantaged older people. The neighbourhood, the small town and the village are introduced as relevant settings for preventive strategies. The fourth part presents the results of a pilot research project on neighbourhood-based health promotion for disadvantaged older people conducted in 2005, showing that successful prevention interventions directly address the needs of specific target groups of older people and rely on participation and empowerment. Supporting social networks emerged as a key issue; organisational networking was also needed to maximize a neighbourhood's resources and to foster sustainability. Instruments for evaluation and quality assurance have yet to be developed and implemented. Finally areas for further research are identified." (author's abstract)
